Important Forms

Medication Consent Form: This form should be printed, filled out, and brought to the school nurse if you would like for your child to take any type of medication while at school. 

Medication in the Clinic

ALL medication must be in the original prescription bottle with the student's name on the prescription label.  Expired medication can not be kept in the clinic and will not be administered to students. 

PLEASE REMEMBER: NO MEDICATION OF ANY KIND IS TO BE TRANSPORTED TO SCHOOL BY A STUDENT. PARENTS/GUARDIANS MUST BRING MEDICATION TO SCHOOL AND ENSURE A SIGNED CONSENT IS ON FILE IN THE CLINIC.

Flu Season is here

Please remember that your student must be without fever for a full 24-hours without the use of fever-reducing medicine (Tylenol, Advil, Motrin, etc.) before they can come back to school after being sick!
